Key Responsibilities
In this role, Dietitians with Seasons Care Dietitian Network will be expected to perform a range of responsibilities geared toward supporting residents’ dietary needs and enhancing their quality of life. Key duties include:
- Monitoring Nutrition and Hydration Risk: Evaluate and monitor the risk levels related to nutrition and hydration for each resident, ensuring they receive appropriate dietary care.
- Conducting RAI-MDS Assessments: Complete Resident Assessment Instrument – Minimum Data Set (RAI-MDS) assessments, which involve evaluating residents’ nutritional status upon admission, annually or when significant health changes arise.
- Prioritizing Dietary Referrals: Respond to dietary referrals promptly to ensure residents’ nutritional needs are managed in a timely fashion.
- Developing and Modifying Nutritional Care Plans: Create, assess and adjust nutritional care plans tailored to each resident’s unique needs. In cases where dietary interventions are required, work closely with the resident, their family members or substitute decision-makers and the interdisciplinary team.
- Conducting Swallowing Assessments: Perform swallowing assessments as necessary to support residents with swallowing difficulties, ensuring they are offered suitable dietary options.
- Nutrition Counseling and Education: Provide guidance and educational support on nutrition for both residents and their families or substitute decision-makers, contributing to informed decisions regarding dietary care.
- Collaborating with Food and Nutrition Managers: Work alongside the Food and Nutrition Manager to enhance the dining experience for residents, fostering an environment that makes mealtime enjoyable and beneficial for their well-being.
- Participating in Care Conferences and Meetings: Join care conferences and relevant committee meetings to contribute insights on dietary needs and progress.
- Reviewing and Approving Menus: Oversee seasonal menus to ensure they align with residents’ dietary needs and, when necessary, design personalized menus that accommodate individual dietary requirements.
- Training Dietary and Nursing Staff: Provide education and training to both dietary and nursing staff on home-specific dietary policies and procedures, supporting staff in delivering consistent, high-quality care.
- Engaging in Quality Improvement Initiatives: Participate in ongoing quality improvement projects tailored to each home, aiming to elevate the standard of dietary and nutrition care offered.
